Neisseria Gonorrhoea

  • Sexually transmitted (mucous membrane to mucous membrane)
  • Non-sexual transmission to newborn resulting in gonoccocal conjunctivitis
  • Starts as uncomplicated infection of lower genital tract but if left untreated will cause complicated infection of upper genital tract
    • prostatitis/epididymitis in men
    • salpingitis or PID in women
  • Sites of infection
    • uretra / enodcervix / rectum / phayrnx / conjunctiva
  • gonococcal infection facilitates acquisition and transmission of HIV

Clinical features

  • 50% of women are assymptomatic
  • Men usually have symptoms
  • Because of less symptms women usually go on to get the complicated infection–> salpingitis or PID

Symptoms in women

  • increased / altered vaginal discharge
  • Lower abdominal pain
  • Rarely heavy menstral bleeding / postcoital bleeding / intermenstrual bleeding due to cervicitis or endometritis

Treatment

  • Ceftriazone 500mg IM with azithromycin 1g orally
