Derm – Examination

Four Important Principles in Examining the Skin

  1. Inspect
  2. Describe
  3. Palpate
  4. Systematic Check

Inspect

  • General Observation
  • Site and number of lesions
  • If multiple: pattern of distribution and configuration

Describe

SSCAMM

  • Size (widest diameter)
  • Shape
  • Colour
  • Associated secondary change
  • Morphology
  • Margin (border)

If describing a pigmented lesion – remember ABCD

  • A – Asymmetry (lack of mirror image in any of the four quadrants)
  • B – Irregular Border
  • C – Colour – two or more colours within the lesion
  • D – Diameter > 6mm

Palpate

  • Surface
  • Consistency
  • Mobility
  • Tenderness
  • Temperature

Systemic Check

  • Nails
  • Scalp
  • Hair
  • Mucous membranes
  • General exam of all systems
